OBD2 report. Cause of my decreased MPG?

Hopefully someone speaks OBD2.

Recently my MPG has dropped from the high 20s to the high teens. I used to be able to get 30+ mpg cruising along on the freeway, now it struggles to get 26. Im not throwing any CELs and the car is not behaving in any unusual way.

I ran this test through Torque Pro on my smartphone. Here are the results. The failed tests are in bold.
