Big problem after changing 3rd/4th gear pressure switches

Rain on and off today so I went to my friend's shop to have a minor front end clunk checked and figured while it was on the lift I'd have him change out the 3rd and 4th gear pressure switches. He calls Honda and they say the part numbers have changed for the switches and sent over the following:

28600-RKE-004
28610-RKE-004
90471-PW7-A00 (2) the washers

Pretty easy swap out it appeared but on the test drive, once it shifted into 3rd gear, it was shuddering and started tossing out codes along with a blinking D light.

He brings it back checks the codes (torque converter and shift solenoid A), resets the TCM and then decides maybe he got them reversed. Swaps positions, same exact results. Takes out the new switches, puts the old ones in, again the same results on a test drive. We both looked to see if maybe there was a connection that was missed but didn't see any.

I looked at the big thread about changing the switches and it appears we got the right parts from Honda. Someone else mentioned having a similar issue after doing their switches but never followed up on how it was resolved.

Any ideas? I've been using this mechanic on and off for about 20 years now, this is the first time a repair has gone wrong.

FWIW, I had my 5AT rebuilt last summer, put about 27k miles on it since. Other than a very occasional and temperature dependent 5 to 4 down shift minor flair (which I was hoping the 4th gear pressure switch would solve) I haven't had any issues.
